Sudden toothache - what is the cause?

Acute toothache can develop suddenly, without earlier symptoms. It usually involves quite intense pain radiating into other areas, such as the face, neck and head. Patients complain of hypersensitivity to temperature changes between heat and cold, as well as to touch. These symptoms are also accompanied by pain when biting and chewing, sometimes with a fever, inflammation and swelling. There may also be some bleeding.

What are the causes of sudden toothache? We discuss them below.

  • Caries – advanced tooth decay is very often the cause of sudden acute tooth pain. If bacteria reach the pulp of the tooth, root canal treatment is required.
  • Tooth cracking or breaking – as a result of trauma, chewing hard foods or existing weakness in the tooth structure caused, for example, by untreated bruxism. A crack or fracture of a tooth usually happens suddenly. In such a situation, we recommend that you report to our Perfect Smile practice right away.
  • Lost filling or crown – this exposes parts of the tooth that are susceptible to acute pain. Unfortunately, without rapid intervention, this can lead to the build-up of bacteria and the development of caries.
  • Tooth abscess – accumulation of pus caused by infection leads to severe pain.
  • Gingivitis or periodontitis – advanced gum and periodontal disease can lead to acute pain in the teeth and surrounding tissues. 

A sudden toothache always requires immediate diagnosis and treatment to prevent further complications.

What to do in case of a sudden toothache?

A strong toothache usually develops when it’s least expected, e.g. at night or during the weekend. If this happens, we recommend that patients contact our clinic immediately. To temporarily relieve the symptoms in the meantime, you should:

  • rinse the mouth with warm salt water to reduce inflammation and remove food debris from the area of the painful tooth,
  • if there is swelling, apply a cold compress on the outside of the face to help reduce it,
  • for temporary pain relief, take an over-the-counter painkiller.
